Personalisation - placing the individual, with their family, friends and networks, at the centre of the decision making-with the aim of people having flexibility choice and control over how they live and the support they have to do this. To ensure that people achieve their full potential.
Dynavour are committed to the personalisation agenda and that good person centred plans are the best way to support people who use the services.
We believe that being person centred is more than just using new words. Current policy is about working in new ways that genuinely put the people we are working with at the centre of decision making about their life, and the services and supports they want and need. This applies to the people we support who may have:
- A learning disability
- A mental health problem
- A physical disability
- Older people who may need support
- Younger people in transition to adulthood
We believe that we have a responsibility with others to:
- Community inclusion – looking outwards with people we work with.
- Jobs and paid work – Real work being a valued option for all citizens, supporting people to this if this is their choice.
- Personalisation – placing the individual, with their family, friends and networks, at the centre of the decision making-with the aim of people having choice and control over how they live and the support they have to do this.
- Housing – Ensuring people have a choice and control over where they live, and seeing an end to institutional services.
- Transition – supporting people to maximise life chances at times of major change, or when people move between services.
